Clostridium tetani

A spore-forming obligate anaerobe found in soil that produces tetanospasmin, an exotoxin causing spastic paralysis (tetanus) by blocking inhibitory neurotransmitter release in the spinal cord.

Clostridium tetani organism

Clostridium tetani is a Gram-positive (purple), spore-forming bacillus (rod). The terminal spore gives it a distinctive 'tennis racket' or 'drumstick' appearance on microscopy.

🔗 The organism's Gram stain and morphology are embodied by the antagonist's appearance: purple for Gram-positive, a rod-shaped body for bacillus, and a terminal spherical head for the characteristic spore, creating a 'tennis racket' shape. Its material is changed to wood to break the incorrect association with rust.

🔬 Identification & Characteristics

  • •Gram-positive bacillus with a distinctive terminal spore, creating a 'tennis racket' or 'drumstick' shape.
  • •Strict obligate anaerobe; lacks catalase and superoxide dismutase, making oxygen toxic.
  • •Forms highly resilient spores that are ubiquitous in soil and animal GI tracts.

🧠 Pathogenesis & Virulence

  • •Primary virulence factor is tetanospasmin, a potent A-B exotoxin (zinc-dependent protease).
  • •Spores from a deep, anaerobic puncture wound (e.g., rusty nail) germinate and produce toxin.
  • •Toxin undergoes retrograde axonal transport from the peripheral wound to the spinal cord.
  • •In the spinal cord, it cleaves SNARE proteins (synaptobrevin) in inhibitory interneurons (Renshaw cells).
  • •This blocks the release of inhibitory neurotransmitters GABA and glycine.

😬 Clinical Presentation

  • •Loss of inhibition leads to unopposed motor neuron firing, causing spastic paralysis and hyperreflexia.
  • •Classic initial symptom is trismus (lockjaw) due to masseter muscle spasm.
  • •Risus sardonicus is a characteristic grotesque 'grin' from facial muscle spasms.
  • •Opisthotonos is severe hyperextension and arching of the back from extensor muscle spasms.
  • •Neonatal tetanus is a severe form often caused by unsterile cutting of the umbilical cord.
  • •Contrast with C. botulinum, which causes a descending flaccid paralysis by blocking ACh release at the NMJ.

💉 Treatment & Prevention

  • •For active infection, administer passive immunization with Tetanus Immune Globulin (TIG) to neutralize unbound toxin.
  • •Eradicate bacteria and halt toxin production with antibiotics (e.g., metronidazole).
  • •Prevention is highly effective with the tetanus toxoid vaccine, which stimulates production of anti-toxin antibodies.
  • •Natural infection does not confer immunity because the lethal dose of toxin is too small to be immunogenic; patients must be vaccinated after recovery.

SPASmTetanospasmin causes SPAS-tic paralysis.
Renshaw Release RestrainedTetanospasmin acts on Renshaw cells to restrain the release of inhibitory GABA and glycine.

🩺 Classic boards tip-off

An unvaccinated migrant worker presents to the emergency department with difficulty opening his jaw and painful muscle contractions one week after sustaining a deep puncture wound from a rusty farming tool, leading to a diagnosis of tetanus.
