Neisseria meningitidis

Gram ⊖Gram-negative, kidney-bean–shaped diplococci

Neisseria meningitidis

Maltose-fermenting, encapsulated diplococcus: dorm/barracks outbreaks, petechial rash, and Waterhouse–Friderichsen.

College dorm barracks

Outbreaks of Neisseria meningitidis are classically associated with populations living in close quarters, such as college dormitories and military barracks. Incidence peaks in infants and adolescents/young adults.

🔬 Identification

  • •Gram-negative, kidney-bean–shaped diplococci; often seen INTRACELLULAR within neutrophils in CSF.
  • •Oxidase-positive.
  • •Ferments BOTH glucose and MALTOSE (gonorrhoeae ferments glucose only — "MeninGococci ferment Maltose & Glucose").
  • •Grows on Thayer–Martin (VCN) chocolate agar; selective for pathogenic Neisseria.

🧬 Virulence

  • •Antiphagocytic polysaccharide CAPSULE — basis of serogrouping (A, B, C, W, Y) and vaccines.
  • •Lipooligosaccharide (LOS) endotoxin → cytokine storm, DIC, vascular collapse.
  • •IgA protease cleaves secretory IgA → mucosal colonization of the nasopharynx.

🫁 Transmission & risk

  • •Respiratory droplets; colonizes the nasopharynx. Outbreaks in CLOSE QUARTERS — college dorms, military barracks.
  • •Terminal complement deficiency (C5–C9 / MAC) → recurrent Neisseria infections.
  • •Anatomic or functional ASPLENIA → risk from encapsulated organisms.

🧠 Disease

  • •Acute bacterial MENINGITIS: fever, headache, nuchal rigidity (± altered mental status).
  • •MENINGOCOCCEMIA: abrupt non-blanching PETECHIAL/purpuric rash + septic shock.
  • •WATERHOUSE–FRIDERICHSEN: bilateral adrenal hemorrhage → acute adrenal insufficiency, DIC, shock.

💊 Treatment & prevention

  • •Empiric treatment: CEFTRIAXONE (3rd-gen cephalosporin); add vancomycin empirically for meningitis.
  • •Close-contact PROPHYLAXIS: rifampin, ciprofloxacin, or ceftriaxone.
  • •Quadrivalent conjugate vaccine (MenACWY); separate serogroup B vaccine (capsule B mimics human NCAM → poorly immunogenic).

🧠 Mnemonics

Meningococci ferment MaltoseN. MeninGitidis ferments Maltose + Glucose; N. Gonorrhoeae ferments Glucose only.
No spleen, no complement → NeisseriaAsplenia and terminal complement (C5–C9) deficiency both predispose to invasive Neisseria.
Close quartersDorms & barracks = classic outbreak settings; droplet spread.

🩺 Classic boards tip-off

College freshman / military recruit with fever, headache, stiff neck and a rapidly spreading non-blanching petechial rash → N. meningitidis. Hypotension + bilateral adrenal hemorrhage → Waterhouse–Friderichsen.
