🔬 Identification
- •Gram-positive, rod-shaped bacterium.
- •Obligate anaerobe, meaning it cannot survive in the presence of oxygen.

☠️ Virulence & Pathophysiology
- •Produces a preformed, heat-labile protease exotoxin that is inactivated by boiling.
- •The neurotoxin gene is encoded on a lysogenized bacteriophage (lysogenic conversion).
- •Toxin cleaves SNARE proteins, which are required for vesicle fusion at the presynaptic membrane.
- •This blockade prevents the release of acetylcholine (ACh) at the neuromuscular junction, causing flaccid paralysis.
- •Some strains produce C3 toxin, which glycosylates Rho GTPases and disrupts the host cytoskeleton.
😵 Clinical Presentation
- •Causes a symmetric, descending flaccid paralysis, starting with cranial nerves and progressing downwards.
- •Initial symptoms ('the 4 D's'): Diplopia, Dysarthria, Dysphagia, and Dyspnea.
- •Pupils are characteristically fixed and dilated (mydriasis) due to blockade of parasympathetic input.
- •Deep tendon reflexes are often preserved, especially early in the course.
🥫 Transmission
- •Adult (Foodborne) Botulism: Ingestion of preformed toxin, classically from improperly canned or preserved foods.
- •Infant Botulism: Ingestion of spores (classically from honey) which germinate and produce toxin in the immature gut, leading to 'floppy baby syndrome'.
- •Wound Botulism: Spores contaminate a wound and produce toxin; associated with black tar heroin injection.
💉 Diagnosis & Treatment
- •Electrophysiology shows a decreased end-plate potential (EPP) amplitude upon nerve stimulation, but a normal miniature end-plate potential (MEPP) amplitude.
- •Adults (foodborne/wound): Treat with equine-derived (horse) botulinum antitoxin.
- •Infants: Treat with human-derived botulism immune globulin (BIG-IV) to avoid sensitization to equine proteins.
💆 Therapeutic & Cosmetic Uses
- •Botulinum toxin (Botox) is injected locally to cause temporary muscle paralysis for cosmetic reduction of wrinkles.
- •Used therapeutically for conditions with muscle overactivity, such as achalasia (relaxes the lower esophageal sphincter), dystonia, and spasticity.
